Minors ages 12 & Under REQUIRE an appointment


ID REQUIREMENTS:

IT IS IMPORTANT TO READ THE ID REQUIREMENTS, IF WE DO NOT HAVE PROPER DOCUMENTATION IT COULD IMPACT OUR ABILITY TO PERFORM THE PIERCING AND MAY RESULT IN A CANCELLATION/NO-SHOW VIOLATION TO APPOINTMENTS


- We CANNOT accept copies of ID’s or documents, they must be original. No photos or photocopies.

- ID’s CANNOT be expired or punched, if they are we will require the papers to go along with them

- We CANNOT accept insurance cards or social security cards as a form of ID for anyone

- Minors MUST be accompanied by a PARENT or LEGAL GUARDIAN - siblings, aunts/uncles, grandparents, ect. can’t be the one signing or for the minor unless they are the legal guardian to the minor

- Please call if you need clarification or help on what identification and/or documents to bring


KIDS/Minor EAR LOBES:

- We can accept either a birth certificate or a passport for the minor (or if they have a state issued ID, follow the requirements listed under “Minor Body Piercing”)

- Parent/legal guardian’s state or federally issued ID (state ID, drivers license, drivers permit, or passport)

- If you bring the minors passport, we will need the parents last name to match the child's last name. If it does not match we will need the child's birth certificate

- The parents name on their ID MUST MATCH how it was printed on the child’s birth certificate. If it does not match we will require the “paper trail” of how it has changed with legal documents

- If a LEGAL GUARDIAN is bringing the minor in rather than a birth parent, we will require the child’s birth certificate as well as any legal documents detailing the rights to the minor


Minor Body Piercing (anything other than lobes):

We NEED a photo ID for BOTH the minor and the parent/legal guardian for anything other than ear lobes

- State or federally issued ID for BOTH the minor and the parent/legal guardian (state ID, drivers license, drivers permit, or passport)

- Last name and address MUST MATCH on the parent and minors ID. If both of those things do not match, then we will require the minors birth certificate

- IF the minor does not have a state/federally issued ID we can accept the CURRENT years physical copy of their school ID, with this we MUST have a birth certificate for the minor!

- The parent & minor’s name on their ID must match how it was printed on the child’s birth certificate. If it does not match we will require the “paper trail” of how it has changed with legal documents

- If a LEGAL GUARDIAN is bringing the minor in rather than a birth parent, we will require the child’s birth certificate as well as any legal documents detailing the rights to the minor


Adults 18+ All Piercings:

- State or federally issued ID (state ID, drivers license, drivers permit, or passport)
